Bring photo Id with your name, picture and address!

Don’t have ID with your address on it? If you live in a Rental, your landlord or other authorized representative can fill out an Attestation Form. If you are in Supportive Living, Long-Term Care, Shelter, or Community Support then Attestations are also available to you. Click here for more information.
